Download and Complete the Application for Minnesota Teacher Certification Renewal.

Submit a completed Conduct Review Statement.
- Have an authorized adminstrator complete the employment verification section of the application.

Attach a check or money order in the amount of $57 for application processing made payable to the "Commissioner, Department of Education".
