Download and complete State Form 9331

An Evaluation fee of $35 made payable to the "State of Indiana"
- A License fee of $35 made payable to the "State of Indiana"
- Photocopies of both sides of an un-expired out-of-state teaching license
- An Official Instructional and/or Educational Experience verification.
- Official Praxis Score Reports sent directly from the Educational Testing Service.

Limited Criminal History Report: Send application to the Indiana State Police Central Respository. Then send the returned, original report with your Indiana teacher certification application.
